- Approach the unit or department Manager to provide feedback or seek assistance.
- If further follow-up is required for patients, visitors, clients additional feedback should be directed to Patient Relations.
- If follow-up is required for employees, physicians, students, volunteers or contractors, additional information and/or feedback should be directed to Workplace Diversity.
- All feedback will be reviewed and forwarded to an appropriate individual or service provider at UHN for follow-up. Where a complaint is received appropriate actions will be taken in accordance with Patient Relations or Workplace Diversity Policy to ensure service equity.
