The Ambulatory Clinics of the Multi Organ Transplant Program are designed to meet the follow-up health needs of the organ transplant recipients. All patients transplanted at UHN have a designated Transplant Coordinator within the Ambulatory program who assists in managing their care along with the multidisciplinary health team. Each organ group has a specific time and weekday for clinic, with appointments scheduled through their coordinator.

The heart transplant clinics are held every Thursday from 8 am until 12:00 pm. Please check with your transplant coordinator in order to make your appointment or schedule a new appointment.
