The Ambulatory Clinics of the liver transplant program are designed to meet the transplant-specific follow-up health needs of all patients of the liver transplant program. All liver transplant patients at UHN have a designated Transplant Coordinator within the Ambulatory program who assists in managing their care along with the multidisciplinary health team. All clinic appointments, except for referrals, are scheduled by the transplant coordinator.
Our program offers three different types of outpatient clinics:
- Patients with liver disease who are referred to us for the first time to determine if liver transplantation is the best treatment option will be seen in the Assessment Clinic, Tuesday (1200-1600).
- Patients accepted on the liver transplant waiting list fwill be seen in the Pre-transplant Clinic Wednesday (1300-1600).
- Patients who received a liver transplant are followed for their transplant related issues in the Post-Transplant Clinic. Post-Transplant Clinics are held Mondays, Wednesdays and Thursdays from 8:30 or 9:00 am to noon.

It is important to come to your clinic visit with a current medication list or know your medications and doses. Bring along any relevant information: record of temperatures, blood pressures (if available), and weight. If you are newly monitoring your blood sugars, bring along your blood sugar readings. It is sometimes helpful to write down questions so they can be addressed when you are in clinic.
Your next clinic appointment is scheduled each time you are at clinic. If you need to change your clinic appointment or if you want to book one earlier than scheduled you can leave a message on easy call for your coordinator.
