Pre-Transplant Clinic
A clinic appointment for patients listed for transplant where the patient may be seen by the transplant assessment coordinator, APN, and/or physician. A systematic review and assessment is completed.
Post-Transplant Clinic
A clinic appointment for patients who have had a transplant where the patient may be seen by the transplant coordinator, APN, and/or physician. A systematic review and assessment is completed.
APN (Advanced Practice Nurse) Clinic
APNs care for patients in both the in-patient and out patient care settings. In the out patient clinics, APNs work collaboratively with other health care professionals. They may see a patient with or with out a physician depending on the patient needs.
