Patient Information/Role

At UHN Patient Safety is very important to us.  We are dedicated to delivering safe, patient centred care in an integrated care delivery environment.  Patient Centred Care (PCC) is about caring for patients by first finding out what their needs and values are – and then tailoring the care we provide to meet those expectations and enhance their experience while in our care. 

What is the role of the patient/ family? 

  • Ask questions and make sure you understand the answers
  • Take a relative or friend with you to help you ask questions and understand the answers
  • Tell your health care professional if you have new or different symptoms
  • Be open about your health goals, history and beliefs with your health care provider
  • Keep track of progress between visits to your doctor and discuss them at your next visit
  • Keep a medication list with ALL medications you take (even the non-prescription medications) and give this to your doctor and pharmacists
  • Get the results of any test or procedure.  Ask when and how you will get results.  Do not assume that the results are fine if you do not get them when expected
  • Tell a member of your health care team if you have ever had a bad allergic reaction to any food or medications
